Objective:
In the skeletal class III patients, maxilla is deficient in three diminutions .This issue relates to hard tissues and also soft tissues and functional spaces .The aim of this study was the evaluation of the changes of airway diminutions after maxillary advancement with tongue guard appliance in the patients with skeletal class 3 and suffering of obstructive apnea in Zahedan.
Methods & Materials:
From 20 patients with skeletal class III malocclusion with maxillary deficiency and obstructive apnea, lateral cephalograms were taken before and after maxillary advancement with tongue guard appliance. Radiographic tracings were done and airway radiographic variables were measured.
 Resaults:
The lower pharyngeal width, oropharynx 2 areas, and SN-CVT angle presenting that craniocervical angulation was not change. Upper and middle pharyngeal width in the airway passage increased at the significant levels. Also, The area of the nasopharynx and oropharynx 1 showed Significant increases. Lower pharyngeal width and oropharynx 2 areas showed insignificant increases. A correlation at a significance level was found between the measurements of the SN-CVT angle and Wits appraisal angle and the oropharynx 2 area.
Conclusions:
Maxillary advancement with tongue guard appliance caused the upper airway dimensions to increase in patients with retrusive maxilla and obstructive apnea.

Key words:
